Records Request

In order to obtain a copy of a Traffic Accident Report, please go to the Police Department. You will need to provide the following:

  • Case Number
  • Accident Date
  • Name of Person Involved
  • Drivers License Number

For accidents occurring in 2026, reports are requested online through CrashDocs. Most are available within 5 to 10 days. If a report is unavailable after 10 days, contact Police at 801-804-4700.

To request a copy of a police report, you must generally be the subject of the record or the legal guardian of a minor named in the record. Active reports cannot be released until the case has been cleared and the report is complete. If you are requesting a report on behalf of someone else, you must provide a signed and notarized third-party release.

Utah law allows up to 10 working days to process a records request. An expedited response may be available in certain circumstances.

To submit a request, provide:

  • Photo identification
  • Date of the incident
  • Description of the record and reason for the request
  • Case number, if available
  • Completed GRAMA Request Form

Fees

  • Police report: $10
