To request a copy of a police report, you must generally be the subject of the record or the legal guardian of a minor named in the record. Active reports cannot be released until the case has been cleared and the report is complete. If you are requesting a report on behalf of someone else, you must provide a signed and notarized third-party release.
Utah law allows up to 10 working days to process a records request. An expedited response may be available in certain circumstances.
To submit a request, provide:
- Photo identification
- Date of the incident
- Description of the record and reason for the request
- Case number, if available
- Completed GRAMA Request Form

- Police report: $10
- CD/DVD: $25
- Color copies: $2 per page
- Certified copies: $2
- Additional fees may apply for lengthy reports.

Completed requests may be mailed, faxed or delivered to the Spanish Fork Police Department. Requests may be faxed to 801-804-4740. Payment is due when the request is submitted. For payment arrangements or questions, call 801-804-4700.
